What data we collect
In order to access the QI Toolkits relevant to your GP practice, we ask you to choose or enter the following information:
- Your practice's name
- Your practice's address, including post code
- Your practice's region
- Your name
- Your e-mail address
- Which computer system your practice uses
Legal Basis
The legal basis for our collection of the information aligns with GDPR Article 6(1)(e), whereby "processing is necessary for the performance of a task carried out in the public interest or in the exercise of official authority vested in the controller".
How we use your information
We use the information listed in section 3, above, in order to:
- Provide your practice with the QI Toolkit which is appropriate to your practice computer system
- E-mail you any confirmation which that process may require
- Monitor uptake of the Toolkits within and across various geographic footprints.
How we share your information
In general we will not share the information we collect with anyone else, other than in aggregate form such as briefing other NHS bodies on the numbers of practices which have accessed the toolkits, or in the case of a legal mandate. Your information may also be accessible by authorised officers of NHS Midlands and Lancashire Commissioning Support Unit, which hosts the web site, in the event of your requiring technical support.
Keeping your information safe
We use high standards of IT security, including encryption and anti-malware protection, to assure the safety of your data.
Your rights
Under the Data Protection Act 2018 and the EU General Data Protection Regulation 2016 you have the following rights:
