Please complete the registration process for the Quality Improvement toolkits. You will be given the option to select which toolkit you would like to download, BPQI or AFQI. This will take no longer than a few minutes to complete.
This Toolkit is currently available for practices in Cheshire and Merseyside and Lancashire and South Cumbria.

Support is available to help you set up the toolkits in your practice.
After downloading:
- if your practice is within Cheshire, Warrington or Wirral, please log your request on our Self-Service Portal*
- if your practice is within Liverpool, Knowsley, St. Helens, Halton or Sefton, please e-mail your request to mlcsu.dataquality@nhs.net
